Your admission checklist

Welcome to Burnside War Memorial Hospital, your not-for-profit community Hospital. We thank you for choosing us to manage your care. We understand that coming to hospital for a procedure/medical admission can be a stressful time and we strive to ensure that you have the best possible experience. At Burnside Hospital, our doctors and staff will work in partnership with you to provide a high level of safe and effective care.

To ensure a smooth and efficient admission process, we encourage you to have all the following listed details complete and / or ready before you arrive at the Hospital.

eAdmission Please ensure your eAdmission online form is complete.
Forms

Please ensure all supplied forms have been completed and signed.

A completed copy of the patient health questionnaire.

Private Health
Insurance

Please bring with you all private health insurance details inclusing your insurers name, your membership number and your current level of cover.

If you have Private Health Insurance you will be required to pay the excess and maximum co-payments as per your estimated length of stay, no later than the day prior to admission. Any additional or out-of-pocket expenses incurred during your stay with us will need to be paid before going home.

 

Medicare Please bring with you a current Medicare card.
Medications

Please obtain an up-to-date list of your current medications from your general practitioner or local pharmacist. Please bring this list with you to hospital.

Please also bring with you all of your current medications (including insulin, supplements and alternative medications)  in the original packaging. Medications must be in the original packaging in your name in order for us to be able to administer them to you in hospital (please do not bring Webster packs or dosette boxes).

  • Your next of kin
  • A contact person in case of an emergency
  • The person responsible for paying your account
  • Your General Practitioner
Other

If applicable, please ensure you bring with you, your:

  • Health Care Card
  • Concession Card
  • Commonwealth Seniors Health Card
  • Safety Net Card
  • DVA Repatriation Card
  • Authority to Admit letter from DVA or WorkCover
  • Credit card for payment
  • Copy of your Power of Attorney or Advanced Health Directive
  • List of questions you would like to ask
  • Pregnancy Health Record (Maternity Patients)
