Submitting your hospital admission form online is a seamless and efficient way to streamline your healthcare journey at Burnside Hospital.

Plus, you have the flexibility to complete your forms at your own pace, from the comfort of your home, making the entire admission process more convenient and accessible.

Embracing this modern method not only contributes to a more eco-friendly environment but also ensures that our staff can prepare for your admission before you even arrive. 

We encourage all patients to utilise the online admissions process if possible. We also kindly request that you please complete your eAdmission at least two weeks prior to your admission.

Hard copy admission forms will still be accepted at both sites, however if you are mailing your forms, please allow adequate time to cater for any postal delays.

If you are mailing or emailing your admission form, please ensure you send it to the correct hospital site for your admission.
