With many benefits to patients as well as to the hospital in terms of reducing manual data entry and our carbon footprint, we encourage all patients to utilise the eAdmissions process.

Hard copy admission forms will still be accepted as normal, however these should be returned at least two (2) weeks prior to your admission.

Admission forms are accepted by post, hand delivery, fax or email at both Toorak Gardens and Stepney sites. Please ensure that you are adressing your admission form to the correct hospital location for your admission.
