eAdmissions is our preferred form of admitting patients. Paper admissions are still acceptable and should be returned at least 2 weeks prior to your admission via one of the following methods:

- Fax (08) 8364 0038
- Post or hand delivery to Burnside Hospital at 120 Kensington Road, Toorak Gardens SA 5065.
If you are attending a preadmission appointment, admission forms need to be received at the hospital at least 2 business days prior to your appointment.
