Submitting Your Admission Form
Submitting your hospital admission form is important when preparing for your visit to Burnside Hospital. We make this process as convenient and efficient as possible by strongly encouraging patients to use our eAdmissions system.
Please complete your eAdmission form at least two weeks prior to your scheduled admission date.
Submitting Hard Copy Admission Forms
If you need a hard copy patient admission form, please download below, ensuring you post or return the completed hard copy to the relevant Burnside Hospital location so that we have it at least two weeks prior to your scheduled admission date.
- Patient Information Pack (Section A - Please take the time to carefully read the information provided about your upcoming stay at Burnside Hospital. It has been designed to help you understand what to expect before, during and after your admission.)
- Burnside Hospital Patient Admission Form (Section B - must be completed and returned as soon as possible so we can confirm your admission and/or book your birth at Burnside Hospital.)
- Maternity Service Patients (Section C - Complete and return to Burnside Hospital in the 34th week of your pregnancy.)
If you're unsure of your admission site, please check with your doctor.
Easy Tips for Filling Out Your Admission Form
- Please use a blue or black pen.
- Return to Toorak Gardens or Stepney (whichever hospital you are being admitted to) at least two weeks before your admission date.

Sleep Centre Patient Admissions
- All sleep centre Patient Admission forms should be sent to the Burnside Hospital Toorak Gardens site.
- If completing your admission form less than two weeks prior to your admission date, please scan and email it to: [email protected] or fax it to: (08) 8331 7152.
- Patient queries regarding an upcoming sleep study or sleep centre admission should be directed to (08) 8202 7272 during business hours (9am to 5pm), Monday to Friday.
Obstetric / Maternity Patients (Toorak Gardens)
Below are the resources for Section C of the admission form, which should be completed during the 34th week of gestation. This section is specifically required for obstetric patients only.
