To maintain our high standards of efficiency, we ask that you please complete your Patient Admission form as soon as possible. This will enable us to prepare for your stay promptly.

If you require help completing your Patient Admission form, please speak to your doctor as they have access to your medical information. You can submit your form at least two weeks before your admission date using our digital e-Admissions system, or return a hard copy to the relevant hospital.

To help facilitate a smooth admission to hospital, please:

  • Obtain a list of your current medications from your GP and bring it to all your appointments and the hospital on the day of admission.
  • Bring current x-rays or scans relevant to your surgery to the hospital on the day of your admission.
  • Follow your specialist’s instructions regarding your admission date and time, fasting requirements, and medication instructions prior to your surgery.
  • Contact your health fund before admission to check your level of cover and clarify any payable excesses or co-payments required.
  • Pay any health fund excess, gap or co-payment, including those applying to basic cover, no later than the day prior to your admission.
  • Request an estimate of the total cost of your hospitalisation if you do not have private health insurance or an accepted Workers Compensation or Third-Party Claim.
  • Pay the estimated total fees prior to admission with any shortfall between the estimated and actual fees due at discharge.
