Your Burnside Hospital Case Manager / Discharge Coordinator 

If you require a period of respite in a Residential Aged Care facility after hospital, our nursing staff will prepare a transfer summary of information for the facility you have chosen, to ensure provision of timely and accurate information about the care received at Burnside Hospital.      

If you are returning home but your care needs have changed, our Case Manager / Discharge Coordinators will liaise with community, aged care and other services providers to ensure a seamless transition from hospital back to the community.
